CPR CLASS REGISTRATION FORM

To register for a CPR class complete the form below:

To register for a class using this form, you must choose a date from the class schedule.

See the public class schedule below to list a date on the form. 

For Healthcare Providers (Healthcare providers, nurses, medical staff/clinicians, people expected to use medical supplies in the event of an emergency)

For Lay Rescuers (non-healthcare related jobs, babysitters, office staff, expecting parents, lay-rescuers)

Private classes are available by appointment only.

To schedule a private class, you must have a minumum of number of students.

PAYMENT DUE AT THE TIME OF THE CLASS.

Payments can be made at Station #1, Administrative Office, 200 E. Route 38, Elburn, IL by cash or check.

Debit or credit card payments can be made by going to the Home page and choose "Make Payment".

Be sure to list your name & the date of CPR class that you are registering for.

In order to hold a class, we need to have at least 4 people registered. If you need to cancel, please call at least 24 hours before the scheduled class.
